With due respect, in connection with the issuance of Student Identity Cards (KTM) for students admitted in the Odd Semester of Academic Year 2024/2025 whose Virtual Accounts are in Bank SUMUT, we kindly request your assistance in informing the respective students in your Faculties to collect their KTMs at Bank SUMUT, USU Sub-Branch Office (KCP) located at Jalan Dr. Mansur, USU Campus, Medan, on the following schedule:

We would like to inform you that the requirements for collecting the KTM are as follows:

  1. Original National Identity Card (KTP) and 2 photocopies;

  2. 1 photocopy of Family Card (KK);

  3. Initial cash deposit of Rp. 100,000 (One Hundred Thousand Rupiah);

  4. Ballpoint pen for filling out forms;

  5. Signature must match the KTP (if the signature differs from the KTP, students must bring a Rp. 10,000 duty stamp, 1 sheet).
